where can I build and download a intake questionaire for a drug and alcohol treatment center for school?
I need to make my own apllication for admintance and hope there is a tutorial to help me build one
Having volunteered at treatment centers and psychiatric circles for many years, the intake documents – even blank – are highly confidential.
But … you know the internet. Could be there is one out there. The usual term is "intake assessment".
